The Fitness Plaza is pleased to announce our self-service option. Through our self service portal you have the ability to purchase memberships, manage your membership details (ie. payment information, personal information, etc.) and sign up for zoom classes on your personal device

Note: Each member must have their own account when using our online self-service option. If the member is under the age of 18, a parent or guardian must create an account for the member and electronically sign any contracts or waivers for the member.

First time users:

1. Follow this link to our self service site: https://fitnessplaza.ezfacility.com

    

2. Select “Login” and create your account.
   Note: Your account will not be created until you check your email and follow the link provided, this is a common error when creating an account.

    

3. Complete your registration. When registering your account there will be three open ended response questions: #1 Fitness Plaza ID, #2 Insurance Provider, and #3 Fitness ID number from Insurance Co. All members with the exception of Tivity members must fill out #1, this number can be found by the barcode of your Fitness Plaza ID card (ex. 00012345). If you joined using a program provided by your insurance, you MUST fill out #2 and #3. Otherwise leave #2 and #3 blank. For examples of what programs we take and what program ID numbers we need, please see the membership tab of our website.

    

4. Once you are logged in you will be able to manage your Fitness Plaza account.
